Contact the Hospital Service Coordinator (HSC) to request
transportation at least one week before the scheduled appointment.
Contact the HSC as soon as possible regarding appointment
cancellations or changes. It is the veteran's responsibility
to have a family member make the contact if he/she is unable to do
so.

You must be dressed and ready to leave for the
hospital at the time specified by the Coordinator.
The Driver cannot wait for
patients who are not ready on time.

DO NOT
request the driver to make side trips to take care of the patient's
personal business.

DO NOT
engage in any activity that will distract the driver's attention.

Wear seat belts at all times. Any passenger refusing to buckle
up will be refused transportation.

Be dressed and ready to leave for the hospital at least fifteen
minutes before the specified pick up time. The driver cannot
wait for patients.

Be able to board and disembark the vehicle unassisted.

Only carry articles small enough to be held on the veteran's lap or
store under the van seat.

Only be accompanied by (a) the spouse, if the spouse has a "Spouse
permission to travel" slip from the patient's doctor, or (b) a care
giver who is authorized by Veterans' Affairs to provide the veteran
with "Aid and Assistance".

You
DO NOT receive reimbursement for travel expenses if the
veterans is utilizing the DAV Transportation Network.

DO NOT
smoke, chew tobacco, drink any form of alcohol, use foul language or
bring weapons, street drugs or any illegal substance on the van.
All trash will be placed in the proper trash receptacle.
